Precision Heart–Brain Care in Heart Failure: Integrating National Linked Health Data and a Randomised Trial to Optimise Disease Management

This PhD establishes a novel translational research framework that integrates national real-world evidence with prospective randomised trial data to optimise precision heart failure care. The program progresses logically from discovery to implementation: 1. Discover high-risk heart–brain phenotypes using Australia’s national linked data. 2. Develop precision risk models that identify patients most likely to benefit from intervention. 3. Validate these hypotheses prospectively within the HF-Outreach randomised trial. 4. Understand the mechanisms through which multidisciplinary care improves outcomes. 5. Translate the findings into a scalable national model of precision HF care. By integrating epidemiology, precision phenotyping, clinical trials, implementation science and health economics within a single research program, this thesis will provide one of the first comprehensive evidence pipelines for precision disease management in heart failure and establish a framework applicable to other chronic diseases.
