Models of care for prevention of cardiovascular disease in women with a history of hypertensive disorders of pregnancy: a systematic review

Description 
Cardiovascular diseases (CVDs) are not only the leading cause of death in women but also contribute significantly to disability. Women with a history of hypertensive disorders of pregnancy (HDPs) have an increased risk of CVDs. It is therefore critical that care pathways are established early after pregnancies complicated by HDPs to mitigate this risk. Models of care are ways in which health services are delivered for individuals and populations as they experience various stages of health and illness. They are designed to ensure that people receive the right care, at the right time, by the right team and in the right place. This project aims to review the available evidence on the models of care for preventing CVDs in women with a history of HDPs and describe the most effective strategies.
